Lady Gaga emerged internationally with The Fame in 2008 and expanded that pop vocabulary through The Fame Monster and Born This Way. Later work in film and jazz demonstrated a career that could move between highly theatrical music videos, live singing and dramatic acting. Changes of costume are part of that movement between performance contexts.
The raw-meat dress worn at the 2010 MTV Video Music Awards remains a documented example of clothing used as a public statement. It should not stand for her entire wardrobe. Compare such deliberately confrontational objects with the more formal tailoring and eveningwear seen at other events: the question is how scale, material and occasion shape a performance, rather than whether an outfit is simply unusual.
